BIRTH CERTIFICATES

Procedure for obtaining Copies:

Appear in person at the Health Department between the hours of 9:00 am and 3:00 pm
Be the person referred to in the birth certificate OR
Be the parent or legal guardian of the person referred to in the birth certificate
Have a birth date after 1942 Earlier birth records not yet available in database
Have Photo Identification
Obtain the form requesting a duplicate certificate.
Pay $20.00 Fee
